Urinalysis 101: What a Urine Test Actually Checks For

A urinalysis is one of the most common tests in medicine, and also one of the most quietly informative. From a small sample, a lab can pick up early signals about your kidneys, your hydration, your blood sugar, and whether an infection is brewing — often before you feel any symptoms. If your doctor has ordered one, or you've seen it on a lab slip and wondered what it's looking for, here's a plain-language guide to what a urinalysis is and what your results mean.

What a urinalysis is

A urinalysis is simply an examination of your urine. Urine is one of the ways your body clears out waste and excess fluid, so its contents act like a status report on several systems at once. A standard urinalysis looks at the sample in three different ways: how it looks, what chemicals are dissolved in it, and what shows up under a microscope.

Why doctors order one

Urinalysis shows up in a lot of situations because it's inexpensive, painless, and surprisingly revealing. Common reasons include a routine physical or wellness check; investigating symptoms like burning during urination, frequent urges, back or side pain, or blood in the urine; monitoring an ongoing condition such as diabetes or kidney disease; a standard step before surgery; and as a routine part of prenatal care during pregnancy. Because it can flag problems early, it's often used as a first look that points toward whether more specific testing is needed.

The three parts of the test

Physical (visual) exam. The lab notes the color and clarity of the sample. Urine is normally pale yellow to amber and clear. Very dark urine can simply mean you're dehydrated, while cloudiness can suggest infection, or the presence of cells, crystals, or mucus. Some foods and medications can change the color, too.

Chemical exam (the dipstick). A thin plastic strip with chemical pads is dipped into the sample, and each pad changes color depending on what it detects. This is where a lot of the useful information comes from. A dipstick typically screens for:

  • pH — how acidic or alkaline the urine is, which can hint at diet, hydration, or certain conditions.
  • Specific gravity — a measure of how concentrated the urine is, reflecting hydration and kidney function.
  • Protein — small amounts can be normal, but higher levels may point to kidney issues.
  • Glucose (sugar) — usually absent; its presence can be an early flag for diabetes.
  • Ketones — produced when the body burns fat for fuel; seen in uncontrolled diabetes, prolonged fasting, or certain diets.
  • Blood — may indicate infection, kidney stones, or other issues, and warrants follow-up.
  • Nitrites and leukocyte esterase — markers that together suggest a urinary tract infection.
  • Bilirubin and urobilinogen — related to liver and red blood cell function.

Microscopic exam. A portion of the sample is spun down and viewed under a microscope to look for things too small to see otherwise: red and white blood cells, bacteria or yeast, epithelial cells shed from the urinary tract, crystals (which can relate to kidney stones), and casts (tiny cylindrical structures that can indicate kidney involvement).

How to prepare

For most routine urinalyses, you don't need to do anything special — you can usually eat and drink normally beforehand. A few practical notes help ensure a clean, accurate result:

  • You'll often be asked for a "clean-catch, midstream" sample, which means cleaning the area first, starting to urinate, and then collecting the sample mid-flow. This reduces contamination from skin cells and bacteria.
  • Certain foods, supplements, and medications can affect results. Large doses of vitamin C, some medications, and foods like beets can change color or interfere with specific readings, so it's worth telling your provider what you take.
  • Menstruation can introduce blood into a sample, so mention it if relevant — your provider may note it or reschedule.

Understanding your results

Results are compared against normal reference ranges, and a value outside that range doesn't automatically mean something is wrong. Mild dehydration, a recent meal, intense exercise, or a supplement can nudge numbers without signaling disease. That's why urinalysis is usually a starting point rather than a final answer: an unexpected result typically leads to a repeat test or a more specific one — for example, a urine culture to confirm and identify an infection, or blood work to look more closely at kidney function.

Some findings are more clear-cut than others. Glucose in the urine, significant protein, or the combination of nitrites and white blood cells are the kinds of results a clinician will want to explore further. The important thing is that the interpretation happens in the context of your symptoms, history, and other tests — not from a single strip in isolation.

When to talk to your doctor

If you have symptoms like pain or burning when you urinate, blood in your urine, persistent back or side pain, unusual fatigue, or increased thirst and urination, those are worth bringing to a healthcare provider, who may use a urinalysis as part of figuring out what's going on. And if you've had a urinalysis with an abnormal result, follow up on the recommended next step rather than trying to interpret the number alone.

A urinalysis is a small, simple test that offers a genuinely wide window into your health — hydration, kidney and liver function, blood sugar, and infection, all from a single sample.
